Our clinical program has been designed to help older adults experiencing cognitive challenges, such as difficulty with attention or memory, due to mild cognitive impairment (MCI) or dementia. To be eligible for reimbursement under Medicare, we require a referral from a physician or nurse practitioner.

The program is covered by Medicare with a signed order from a physician or nurse practitioner with a qualifying condition. We are currently accepting traditional Medicare and plan to accept Medicare Advantage plans in the next few months.

A Brain Health Assessment happens at the start of Moneta’s program to guide the development of your personalized plan for cognitive rehabilitation therapy.
The assessment helps your therapist to evaluate potential deficits that affect cognition or communication, such as short-term memory or attention span. If you received a referral from your primary care physician or another provider, your therapist would consider the information shared about previous diagnoses. Your therapist will also assess your medical history to understand if there are factors in your life that could be affecting your cognitive function.
Your therapist will determine the appropriate length of your program based on your needs, goals and progress, and your reimbursement coverage. Participation in the program is typically 30 to 90 days. After the program ends, we will provide you with at-home activities to maintain your progress, including monthly check-ins with our care team.
